1) Disciplinary Rule 22 (Season 2012/13): In that between 27th December 2012 and 15th January 2013, both dates inclusive, you placed bets upon 20 football matches.
2) Disciplinary Rule 33 (Season 2013/14): In that between 22nd September 2013 and 5th April 2014, both dates inclusive, you placed bets upon 268 football matches.
3) Disciplinary Rule 31 (current): In that between 2nd November 2014 and 3rd March 2015, both dates inclusive, you placed bets upon 225 football matches.
Outcome: The player was found in breach and a six-match suspension imposed (two matched served immediately and four suspended until the end of season 2015-16 for any subsequent breached of Rules 22, 33 or 31).

Disciplinary Rule 200: In that at the above match you committed an offence listed in the Schedule of Offences contained in Section 3 of Annex C to the Judicial Panel Protocol, namely A2 Violent Conduct, by head-butting Jamie Hamill of Kilmarnock FC
Outcome: Referral rejected. No sanction imposed.
